ALSA will never use /dev/dsp. /dev/dsp is an OSS thing.

On most cheap soundcards, yes, you can use only one program to access
it. Except if they are using a sound daemon (like Arts) that is mixing
the sounds before directing them to the soundcard. Notice that
gnomemeeting doesn't support output to sound daemons because they are
not designed for VoIP and add latency.

I don't understand the question, but if you are launching ESD, then
GnomeMeeting won't use it anyway, and it will block the soundcard for
use with Wolfenstein. If you are launching ESD and artsd at the same
time, then you have a big problem as they are sort of mutually
exclusive.

The problem is that I don't understand what you are doing and what you
are not doing and what you want to do. In all cases :
- you won't be able to have gnomemeeting in a communication and play
wolfenstein at the same time
- but you can play at wolfenstein then use gnomemeeting and the reverse

If it doesn't work, then you have something weird going on:
- a process keeping the soundcard busy (ESD, ARTSd, ...)
- a buggy driver
