Menus - I dont' get gnome2 (reprise)

Ok, how do I change a menu, add a catergory or even a single item?  

I would prefere to be able to do this globally but even on a per user
basis would do at the moment.

Currently I have about 6 apps in the dropdown for the "run program..."
menu that I really want to place in the menus.

I have tried the nautilus approach but now have tow unammed folders in
my menu tree and have crashed nautilus twice.
