Testing the GGV, take 7

at the usual place:

fixes the %%EOF problem that occurs because some programs (like mpage)
don't obey adobe's DSC conventions (adds a configurable "Respect EOF"
option) and, hopefully this time for real, fixes all the media
recognition bugs that were still left crawling around. now it should
properly take into account (in the following order): page media,
document media, page bounding box, document bounding box or - if none of
the above were specified in the document - the user defined fallback
media.
