this fixes a number of annoying bugs in applying preferences and syncing
menus to selected options. it seems that whoever was the last one to
change this did not read the code before changing it and did not test
enough.

also, the policy for per document media and orientation settings in
menus has changed: now they are only respected if the "override *"
menu item is checked.

it also features a new checklist for page from Jonathan Blandford. this was
sorely needed.
