Pine is a poor example. Pine doesn't use an index to read its
file effiently. If you're going to have a file act as an achive, it's
stupid not to have at LEAST a FAT. Sure, you sometimes have to rewrite
the file from scratch, but this is problem with many well worked out
solutions.
