It's a mouse where the middle button is a wheel.  A typical
application would be scrolling in Netscape: I can move my finger down,
turning the wheel, and the page moves up, scrolling down the page.
The wheel is typically some kind of rubber, so it's pretty easy to
use.

On the one I've got, you can also press the wheel to get a middle
mouse button event.  That's not as convenient as pressing an ordinary
button, but the extra functionality of the wheel is worth the
inconvenience.  I miss not having a wheel when I'm at work!
