I am not sure there is a "standard" way, but the most commonly used one
(or so it seems to me) is
DocBook 4.1.2 XML
Definitely you need to include version number and whether it is XML or
SGML; whether you write
DocBook XML 4.1.2 or DocBook 4.1.2 XML or XML DocBook 4.1.2 is, INHO<
not too important.
