I think that the idea of another button was rejected early on.
However, hopefully some devs are still reading: What if the file dialog was modified into a tabbed interface instead, so that it hitting <Ctrl>+S brought up:

One big downside of the current implementation is that if I choose the wrong "save" option I usually don't realize it until I've already navigated away and hitting <Cancel> forces me to navigate again. Aside from that issue, the current philosophy is better and I think this might help solve both complaints.
There may be other ways to combine the two functions into a single dialog while still making it abundantly clear that there is a distinction; this is just one idea.
