I'm not aware of any way to customize *how* birthdays are displayed,
only *if* they are displayed.

I'm using Debian Squeeze and evolution from the official repository 
(evolution (2.30.3-5)

That is really old.

I know. :D 
But it runs and according to your answer the *how* preference is independent of the version.
