A couple of months ago I complained:

and we never got to the bottom of it.

I tried Googling again today and turned up

which pointed me directly at the problem.

Fixing the defaults tabs on my accounts removed the issue completely.
No idea how they had been set incorrectly but I'd doubtless screwed up
somehow.
