Sounds a bit strange.

All i can think is that it couldn't open the outbox and fell back to
inbox, but the code just doesn't work that way.

Where there any other outages at the time?  Could you submit a bug
report on bugzilla.ximian.com to track this?  Set it to Major I guess.
