Reviewing old pre-evo mail I've imported, I see that this also gets used
in From names, so I'd guess this would be needed anywhere in the header
where free-form text is allowed.

Here in germany we often use ISO-8859-1 for enconding "umlauts", so many
from-headers look like this:
From:   Uwe L=?ISO-8859-1?B?/A==?=bbermann <uwe luebbermann deleted>

And on a reply it looks like this:
To: Uwe L=?ISO-8859-1?B?/A==?=bbermann <uwe luebbermann ipremium de>
Subject: On Mon, 2002-02-04 at 10:17, Uwe L=?ISO-8859-1?B?/A==?=bbermann
