Hi Brian,
The actual functionality that I most need is more along the lines of
creating PDF forms to allow for automatically compositing a PDF document
with field values from a database or web form.
What kind of tool were you thinking of for putting the forms together?
Scribus could be a good choice, as it has the layout tools for the form
designer to use, plus the PDF export capability, and embedded
Javascript-in-PDF support:

So the work needed would be to implement Javascript features in Evince
which support the PDF form scripting in Scribus, followed by some
Javascript to pull in the field values from a web/database server.
