Refactored Friends of GNOME forms

Hey folks,

I wanted to inform you that I refactored the Friends of GNOME page. Up 
to now, the wordpress content contained PHP code which I moved to a 
theme file. I think that code should not belong in the wordpress. Now we 
got rid of a lot of redundancy and making updates to the forms is a lot 
easier now.

Everything should work as normal. And you should see no difference on http://www.gnome.org/friends/
If you see anything not working anymore, please tell me.
