Try looking for that picture in ~/.cache/media-art/ and
~/.cache/media-art/64/. If you order by date then you should find the
duplicate that was created and used by the "unknown" album. I've just
had it happen when I played a video and it loaded the first pic it could
find, and I think that should fix it.
