This is an open bug.

When you disable cover art fetching, it only prevents the downloading
of cover art on import. When you play a track or album, regardless of
the state your cover art plugin, Banshee downloads the artwork.
Banshee however also extracts the embedded cover art to a separate
file which is not used for display. So when you play something, you
are left with two images:
artist-album.cover (extracted file)
artist-album.jpg (downloaded file)
If you're using Linux, see comment #12. I added a bash script that
you run which deletes all the jpg files, and copies the extracted
cover files over to .jpg's. This way Banshee displays the correct
artwork.

If you don't use Linux, the script can be ported to other
environments, so just let us know. I hope the bug will be addressed
at some point though.
