[PATCH] Balsa-extern

Hi! 

Here's my balsa-extern patch. It adds an address book type 'extern',
	making it possible to use an addressbook just like mutt's
	'query_command'. It has been tested with lbdb(Little Brother
			Database).

Comments and feedback welcome!

The patch has been made against balsa 1.2.0.
