[xslt] Duplicate definitions - LibXSLT bugs

The other day, I tested a couple of processors with regard to duplicate
definitions. I published the result on XSL-List.

There are three bugs in LibXSLT concerning duplicate definitions of:

* xsl:param
* xsl:namespace-alias
* xsl:attribute-set

Very simple test cases are contained in the posting.
