I've combined the library into the main xslt file - same problem;
further the top of the xslt file is:
<xsl:transform version="1.0"
    xmlns="http://www.w3.org/1999/xhtml";
    xmlns:xhtml="http://www.w3.org/1999/xhtml";
    xmlns:xsl="http://www.w3.org/1999/XSL/Transform";
    xmlns:date="http://exslt.org/dates-and-times";
    xmlns:str="http://exslt.org/strings";
    xmlns:func="http://exslt.org/functions";
    extension-element-prefixes="func date str"
>

and has the correct extension-element-prefixes entry
