Indeed, I've added those to [1], which also contains some other
worthwhile suggestions from you.

Aren't 12 px correct?

Pressing enter resp. escape issues the expected actions (confirmation
vs. cancellation), so what's wrong with these accelerators? Cancel/OK
are stock items, so these should be kept consistent, right?
