Yes, I think there's a good case for Esc being a
shortcut for the Close button, as well as CTRL-W.

Are there any dialogs with neither Cancel nor Close
buttons?

Are there any dialogs with both Cancel AND Close? 

The two seem to be mutually exclusive to me.
