[Tracker] Changelog and README needs some love

Hi,

Here are two requests to Tracker devs:
* please have a Changelog entry to each commit you make
* please consider changing the README in case of relevant changes (EG
feature additions and configure.ac changes)
* the manpages could benefit too from some attention
