The shuffle icon comes from gnome-icon-theme.  The problem is this:
$ find /usr/share/icons -name '*shuffle.png'
/usr/share/icons/gnome/16x16/stock/media/stock_shuffle.png
/usr/share/icons/gnome/24x24/stock/media/stock_media-shuffle.png

Rhythmbox asks for the stock_shuffle icon, so it gets the 16x16 version,
regardless of the size it actually wants.  This has been fixed in
gnome-icon-theme in CVS.
