My suggestion would be to turn on debugging in Orca:
https://wiki.gnome.org/Projects/Orca/Debugging
If this doesn't reveal an obvious issue (e.g., something not installed),
then I would upgrade to a known working distribution/environment that is
closer to what is currently being developed.

Hi Peter,
I also use Orca master, gnome-shell 3.28.4 on Ubuntu 18.04 but cannot
confirm. Maybe restarting gnome-shell can be of help?
