Hi,
I can file a bug but I don't know how to create a test case that can
demonstrate the bug since the application in question runs on a local
network.

Could you please tell me what happens if you use Alt+F6 instead of Alt+Tab?
If pressing Alt+F6 causes Orca to announce the dialog, that suggests to me that you were not in the dialog
before. And if that's the case, that should be filed against Firefox. It's not an accessibility bug per se:
The dialog should be automatically active.

Here at work we use trac, http://trac.edgewall.org/ to track tickets.
Every time a ticket is opened or modified, those involved receive an email describing the changes and a link
to the ticket.
If I click on the link, an authorization dialog will appear asking for a login and password but orca does not
read anything.
I need to switch windows and go back using alt + tab to get the dialog announced by orca. The odd thing
is that if I copy the link and paste it into FF's address bar everything works normally.
Another interesting point is that the problem only happens if FF is already open. If FF has not been opened
yet and we click the link inside the email,
FF is launched and everything works normally.
Since this is an internal application I cannot publicly provide a test link, but I can capture a debug by
reproducing the issue or provide a PVT link.
