It would have to be 

export QT_ACCESSIBILITY=1

(with lowercase 'e', and '_' instead of '-')

But it should have already been done by the at-spi2-core package
scripts. To make sure what could be going wrong, you can try

git clone https://salsa.debian.org/a11y-team/check-a11y
cd check-a11y
./troubleshoot

and send us the output.
