I've noticed issues like this. Have you tried pressing alt+super+s a
few times to toggle orca off and back on? If that doesn't work, open a
run dialog by pressing alt+f2. Type killall orca (this will not kill
the orca that's running on the login screen) and then press enter.
Press alt+super+s  a couple of times to toggle orca off and back on.
If this works, gnome should now autostart orca after login. This is a
bit flaky though, so although orca will usually autostart, sometimes
the keyboard shortcut won't toggle orca. I'm not sure why this is.
