That sounds like this Gtk+ bug, which I'm working around in Orca:

The workaround is in both Orca 3.16.2 and master. So what version of
Orca are you using?

I'm having a problem with orca and gnome-terminal in ubuntu 15.04
(vivid). It's similar to a problem I brought up a year ago with debian
squeeze. If you switch between tabs in gnome-terminal, orca does not
seem to refocus on the new tab. If you press control+tab to switch to
another window and then go back to gnome-terminal, then orca can read
the screen.
If I could get orca to do whatever magic it does when you switch to
another app and then back without having to switch to another app and
back, I'd be okay.
