I had a quick look around about this. My memory seems to have served me 
correctly. Have a look at this bug 
https://bugzilla.gnome.org/show_bug.cgi?id=309015, look towards the end 
for what was actually done towards the end of 2010.

A bit further exploration by me leads to my system providing some sort 
of accessibility of the document should I open evince and then open a 
PDF rather than pass the file in from the command line. I said some sort 
of accessibility, I still don't get caret navigation but flat review 
sees it and when it gets focus orca will read the content but no focus 
navigation.

I noticed also a few error messages about ATK appear in the terminal for 
either way of opening a document.

So, I probably will report these issues to the evince developers but 
before I do I want to know whether others see these issues or not. 
Equally I would like to know if anyone has it working perfectly.
