As an example of what doesn't read:
* Desktop icons (eg. "Computer", "Mike's home", "Waste Basket", etc).
* Gnome's menu's: "Applications", "Places" and "System".
* The gnome run dialog when I press alt_f2.
When I launch applications they seem to be fine for accessibility, its
just more the core desktop features which don't work.

What exactly isn't reading for you? Applets are likely not working; I
think that we need an equivalent to the old libgailgnome code.

had thought it was only gnome-terminal windows it was reading but
that was a mistake, it seems to be any application I start once gnome
has started (eg. I am writing this email using at-spi2 and thunderbird).
This seems to be like my past experience when trying some of the
lighter weight desktop systems like LXDE or XFCE where at-spi is
started later than it possibly should be. So my question now goes:
How is at-spi2-registryd started? What could I do to try and get it
to be loaded earlier?
