A fresh install or an update?

If it was a fresh install, the accessibility profile was enabled?

i had problems similar to what you're describing and and they were solved after I remove pulseaudio.

I am not sure if it will work for you, but I think that you can try.
Firstly make sure that the file ~/.pulse_a11y_nostart is present.
If not, you can create executing the following command:

touch .pulse_a11y_nostart

Now remove pulseaudio executing:

sudo apt-get purge pulseaudio

Finally reboot your machine, only the GUI environment.
