From what I can tell there is no significant accessibility issues with
the GUI for linphone although I personally would use the cli, mainly
because I prefer a cli in general.

Absolutely, but plan to use the Gnome Terminal -- or a Linux console.
Look at linphone and twinkle, both fully accessible from the cli. Use
http://vitelity.net for your PSTN gateway and/or full service provider.
There are other gateways/service providers.
