You'll have to check orca out from svn, build and install it. You also
need to build and install some other libs out of svn such as gail, atk
or tk (not sure which one), and at-spi.
All this info can be found on:
http://live.gnome.org/Orca/DownloadInstall
Once you have installed it an update is as simple as:
cd to_orca_trunk
make clean
svn up
./autogen.sh
make
sudo make install
Every now and then just svn up the other libs you have installed and
recompile... That page should help you though.
