As far as my explorations have shown, this is a problem in the dialog boxes not shared with Firefox. I have filed a bug Here:

I've been checking every button and many of the controls in every dialog box
in Thunderbird.  At present, no hot keys are spoken by Orca when KPD-Enter
is pressed.

I'll file an Orca bug for this in the near future.

I just checked in the source file of the spell check dialog, and the
buttons, labels etc., definitely all have accesskeys defined.

I just did a comparison on Windows, and I definitely see them here.
