Looks like a driver issue. From the log:

The driver didn't initialize the device completely, and NM can't get
the MAC address.
My suggestion is to update the driver or report the to kernel upstream
if updating
driver doesn't work for you.

But why it work after I restart NM? And it worked earlier, before I upgraded to 0.9 version? Why NM resets mac-address of my card (

on first start and didn't do it after restart?

 
I don't know why it only happens with NM 0.9. My friend encountered a similar issue before. He also used
a Ralink wifi card. The driver took a few seconds to initialize the hardware, and NM cann't get MAC
address immediately. His solution is updating the Ralink driver and firmware. Though it's one or two years
ago, I think you can try to update the driver and see if this works.

Ok. I'll try
