It looks like your modem is actually crashing and rebooting itself.
Note how it appears to drop off the bus completely, and then have to get
re-probed and initialized.

1) what specific Huawei modem is this?
2) what firmware version is the modem running?  Older Huawei firmwares
are known to be problematic; you may want to check and see if there's an
update for your device through your provider and apply it.
