Connect To Server Redesign

Hi all,

There are a few UI issues with the current connect to server dialog, so
I recently set about doing a redesign. I've uploaded the results to the
wiki [1].

Thanks to Hylke and Andreas for giving me a hand with this during
GUADEC. :)
