The correct URL is http://foundation.gnome.org/membership/application.php

In fact, there's a redirect from
http://foundation.gnome.org/membership-form.php to
http://foundation.gnome.org/membership/application.php but it's
not working because the server doesn't accept request for php pages
(application.php being an exception).
