[libxml++] namespace support

I have committed some namespace support to cvs - a simplification of the
patch from Dan Dennedy. The actual change is shown in my patch here:

This is demonstrated in examples/dom_build.

I've never really used namespaces in XML, so this API might be
inappropriate, or the API might not do what people expect. Please do
look at it and try it.
