for updated document.  I answered your points Murray, and cleaned up the
comments.  There are some niggly details to formatting the code snippets
that I'm trying to work out.

The wiki should have version control, but probably not the nicest to
use.  If the wiki is useful, I'll create id's for everyone.  If you want
to use LaTex or DocBook or whatever, that's also fine by me.  I'm easy.
