MenuBar without using the UIManager

 There's no documentation of how to create a MenuBar without the UIManager or
a little about manipulating MenuBars created with the UIManager.
 I can create the Menu using the Menu class and the MenuItem class that
includes the MenuShell class, but how can I add all the created menu to a
MenuBar and show it?
