Hmmm, I see. But that’s
strange because on the GTK themes screenshot I looked at, the title bar seems
to be changed also. How does it work?

You are not right :) In Gnome, it is themed by metacity window manager
by default. However it can be any other window manager that is running at the
time. (xwm, icewm,...). On OSX (with or without Apple X11) native window
manager is used and so on...
