LD_LIBRARY_PATH is a run-time thing, not compile-time. Try:

   gcc fred.c `gtk-config --cflags --libs`

then

   LD_LIBRARY_PATH=/opt/xfree86-4.2.0/lib ./a.out

(this is not very portable! eg. HP-UX, Mac OS X, etc. all do this 
differently)

I don't know how great a solution this is :-( Instead you might consider 
linking statically against X, adding the xf86/lib to your ld.so.conf, 
setting LD_LIBRARY_PATH in your .bashrc, or installing X somewhere more 
commonly used.
