There is some special code in gtk-doc for GTK+/GDK/GLib since that was
all gtk-doc was originally aimed at.

Once others started using it to document other libs etc. I started
adding
the '<include>' stuff, but I think there was a bug in it.

I did fix it, but I may not have committed it to CVS.
The released gtk-doc is quite old.


(There is a slight problem in that the gtk-doc used to build the docs
on developer.gnome.org is a forked version, slightly modified by Dave
Mason
& Federico, and I haven't been able to see what changes were made.)
