24 small bugs

Here are some open bugs that I think should be easy to fix - most of
the work for many of these should be creating an appropriate test
case.

They are grouped by approximate priority:
 
 High:   I think we need to fix this before 2.0.0
 Medium: I think we should fix this before 2.0.0
 Low:    It might be nice to fix before 2.0.0

If you want to work on any of these bugs, assign it to yourself, and
if you attach a fix to the bug, please:

 a) put the PATCH keyword on the bug
 b) put it on the 1.3.14 milestone
