App menu disabled Win10

Do you know why in Windows 8.1 and 10 some times I have the App menu
desable? No menu item is enable.

Do I need to check the associated app action?

The issue is rare because some times it is present and some others no. For
example I have a Devel VM win8.1 where the menu is disable but other
instance for tests runs without problems. Now I have a VM with Win10 where
the menu is disable and now I've created a devel install Win10 with the
same issue.
