Can you clearly reexplain what is your exact problem?
I'm puzzled by your explanations, sorry...

What are you trying to do and what fails?

And yes, if you are using a gatekeeper it won't translate the IP
address, but it will proxy the streams. But when you told me that the
calls were coming with a private address, I thought you were really
talking about incoming calls, in which case you can not do anything, not
that you were talking about outgoing calls.
