Best file a bug. A small screencast of the reproducer would also be useful.
Could be a volume problem, in which case the easiest way to make
sure that you're in a good initial setup would be to remove all the files in ~/.pulse/.

Do you mean screenshot - see attached? Note that the volume level is OK
as I can hear music and the default sounds without problems.
Did you install sound-theme-freedesktop?
You'll need a sound theme with sounds with names like:
audio-channel-front-center.oga
audio-channel-front-left.oga
audio-channel-front-right.oga
audio-channel-rear-center.oga
audio-channel-rear-left.oga
audio-channel-rear-right.oga
audio-channel-side-left.oga
audio-channel-side-right.oga
Otherwise, no sound to play...
Ah-ha - another undocumented run-time dep :)
