I'll agree with Kenneth Daniel here.
Gnumeric uses

	Gnumeric <version>

	<changes>
	---------------------------------

The advantage of this sort of a format is that the only agreement
necessary is on what form the version delimiter should take.
Something like 'any line consisting of 40 or more consequtive - or =
seems pretty universal.
