Excellent question!

I had a quick look and it looks like documentation support works
(basically) like this:

 - read help/Makefile.am
   - take DOC_MODULE
   - take DOC_INCLUDES
 - check help/C/$DOC_MODULE.xml exists
    -> it should check for index.page
 - call xml2po with $DOC_MODULE.xml and the files mentioned in
   $DOC_INCLUDES
    -> it should call xml2po and index.page and $DOC_INCLUDES (for
       legal.xml) and $DOC_PAGES

I just filed http://bugzilla.gnome.org/show_bug.cgi?id=589965 with a
patch; hopefully a Damned Lies developer will soon check this.
